Extra Care Registration Form

Our Extra Care Program is available for your child before class starts from the hours of 7:00 a.m. - 9:00 a.m. and after class ends, from 1:00 p.m. - 5:30 p.m. Upon receipt of this completed application, along with the required Registration Fee of $150.00 (Non-refundable), your child will be registered for the 2024-2025 school year Extra Care program. This form must be completed, and the registration fee must be paid by the first time your child attends the Extra Care Program.
