CHICK-FIL-A WEDNESDAY RSVP

PLEASE READ BEFORE SUBMITTING THE RESERVATION BELOW!

With this ticket, your child will receive Chick-Fil-A on the upcoming Wednesday!

One ticket per child. Each child must have their own ticket.

We recommend having your RSVP for the upcoming Wednesday completed by 1PM on Monday prior to secure reservation.

We will pre-order Lunch from Chick-Fil-A through their school catering program for lunch on Wednesdays.

We will serve the child a Chicken Nuggets (5) meal with applesauce, Fruit Snacks and Capri Sun

The cost for lunch is $10.00.

Lunch will be served during your child’s normal lunch time.

I will bring $10 in cash on the Monday prior as a one-time payment. I will sign my child up for Chick-fil-A only on the days when I would like them to participate.

PLEASE READ: If you are signing your child up for Chick-fil-A one time, you must submit a new sign-up form each time. The form must be submitted by Monday prior to the upcoming Chick-fil-A Wednesday.

Same-day Chick-fil-A requests will be subject to a $5 late fee.

If you are signing your child up for Chick-fil-A for the entire school year, you will be billed monthly with your tuition for each Chick-fil-A Wednesday. You will not be charged for weeks when Chick-fil-A is not offered.
